Before and after your gathering
Your reflection starts here.
Use the official evaluation form for both parts of your session feedback.
Before the session
- Open the form and sign into the Google account you will use during the gathering.
- Select the appropriate workshop and complete its pre-session questions, following the instructions in the form.
- Keep the post-session questions for the end of the gathering. Before closing, look for “Draft saved” near the top.
During or after the session
Reopen the same form using the same signed-in Google account. Complete the post-session questions and submit the finished response.

Google Forms normally saves unfinished responses for 30 days when you are signed into a Google account and online. Draft saving must be enabled for the form. Using the same email in an answer field alone does not restore a draft; use the same Google account.
If the form says “Saving disabled,” do not rely on draft saving. Keep it open and contact the facilitator for guidance. Form changes or access changes can affect saved answers. Follow the form’s instructions if they differ from the general steps here.
The form’s own save indicator confirms whether a draft is being saved. Check it each time.
Use ordinary program feedback. Keep patient information, confidential records, account credentials, and identifiable private stories out of open-text answers.
